Used to obtain a list ESTAB requestor's mailing addresses stored on the DOC RDMS database.

After selecting an Address on the CC ESTABAL screen this screen ESTABAE displayed. Then we entered "0011" into the extension portion of the Zip Code field, and enter "U" in the Desired Address Action field. Then, transmit screen to complete request.
